About This Program Heart to Heart Connections is a volunteer community program created by Madison Giaramita as a Girl Scout Gold Award project. The program is operated and administered by The Social Brain, a nonprofit organization based in Farmingville, NY. The Social Brain assumes full organizational responsibility for the administration, operation, and ongoing management of the Heart to Heart Connections program. Heart to Heart Connections is not a standalone legal entity.
The program connects licensed nurses with families in need of care. The Social Brain does not employ nurses, guarantee care outcomes, or operate as a licensed healthcare agency.
Information We Collect When you submit a form on this site, we may collect your name, phone number, address, date of birth, diagnoses, medications, care needs, certifications, NPI number, and resume. This information is collected solely to facilitate connections between nurses and families through the Heart to Heart Connections program.
How We Use Your Information Your information will only be used to match caregivers with families in need. We do not sell, rent, or share your personal information with third parties. Access is limited to authorized Social Brain program staff only.
Data Storage & Security Submitted information is stored securely through our website platform. While we take reasonable precautions to protect your data, no digital transmission is 100% secure. By submitting a form, you acknowledge this limitation.
Health Information Notice This program collects sensitive health-related information including diagnoses and medications solely to facilitate care matching. The Social Brain is not a HIPAA-covered entity; however, we treat all health information with strict confidentiality and use it only for its intended matching purpose. This information will never be shared with third parties without your explicit consent.
Volunteer Disclaimer Nurses who participate in Heart to Heart Connections do so on a voluntary, unpaid basis. No employment relationship is created between participating nurses and The Social Brain or Heart to Heart Connections. Nurses are solely responsible for maintaining a valid, active New York State nursing license and operating within their licensed scope of practice. The Social Brain does not verify, guarantee, or assume liability for the clinical actions or decisions of volunteer nurses.
Family Disclaimer Families who connect with nurses through this program are encouraged to independently verify nurse credentials and licensure through the NYS Office of the Professions at op.nysed.gov. The Social Brain and Heart to Heart Connections facilitate introductions only and are not responsible for care outcomes, injuries, or disputes arising from private care arrangements made through this program.
Girl Scout Gold Award Notice Heart to Heart Connections was originally developed by Madison Giaramita in pursuit of her Girl Scout Gold Award. Madison is recognized as the founder and creator of this program. All ongoing operational, legal, and organizational responsibility rests solely with The Social Brain. Madison Giaramita bears no personal legal liability for the operation of this program.
Limitation of Liability The Social Brain, Heart to Heart Connections, its staff, volunteers, affiliates, and program founder Madison Giaramita shall not be held liable for any damages, injuries, or losses arising from participation in the Heart to Heart Connections program, to the fullest extent permitted by New York State law.
