Introducing Heart to Heart Connections: Bringing Nurses & Families Together
A Girl Scout Gold Award Project in Partnership with The Social Brain
At The Social Brain, our mission has always been to bring people together and strengthen our community. That’s why we’re proud to partner with Girl Scout Madison Giaramita on her Gold Award project, Heart to Heart Connections, a community matching program designed to connect licensed nurses with families in need of skilled home nursing care right here on Long Island.
What Is Heart to Heart Connections?
Heart to Heart Connections is a free, community-driven program that bridges the gap between two groups who need each other. On one side are licensed nurses looking for cases, skilled, compassionate caregivers ready to make a difference. On the other are families caring for loved ones with complex medical needs who need reliable, qualified nursing support at home.
This isn’t a staffing agency. There are no fees, no contracts, and no middlemen. It’s neighbor helping neighbor, backed by the organizational support of The Social Brain.
About the Event
On Saturday, June 13th from 11 a.m. to 1 p.m., Heart to Heart Connections will host its in-person signup event at 20 Wireless Rd in Centereach, NY. This is your opportunity to come in person, learn more about the program, and take the first step toward making a connection.
Whether you are a nurse looking for meaningful work in your community or a family navigating the challenges of home-based skilled care, this event was created for you. Attendees will have the chance to meet program organizers, ask questions, fill out intake forms, and begin the matching process in a warm and welcoming environment.
Families caring for loved ones with needs including lifting assistance, bathing, incontinence care, trach care, GJ tube feeding, and suction machine operation are encouraged to attend. Nurses of all specialties, RNs and LPNs, are welcome.
Why It Matters
Finding qualified nursing care for a family member with complex medical needs can be overwhelming. At the same time, many nurses are looking for meaningful cases where they can build real relationships and make a lasting impact. Heart to Heart Connections exists to close that gap, creating better outcomes for patients and more fulfilling work for nurses.
As Madison puts it: Stronger together. Better care for all.
